Patient Rights and Responsibilities

Non-Discrimination Policy
In accordance with our values of dignity, integrity, service and compassion, it is the policy of the medical center to admit and to allow all patients impartial access to treatment that is available or medically indicated, regardless of race, color, creed, age, political or religious affiliation, sex, sexual orientation, national origin or sources of payment for care.

Any patient who feels that he or she has been discriminated against may file a grievance with a patient representative at 612-273-5050. The patient representative may be contacted by any patient who believes he or she has been discriminated against in any aspect of services on the basis of disability. Any patient may also choose to file a grievance with the Office of Health Facility Complaints (Suite 300, 85 East 7th Place, St. Paul, MN 55101; phone 651-201-4200) or the Federal Office of Civil Rights (Region V, 300 S. Wacker Drive, Chicago, IL 60606; phone 312-353-5693).

Patient Bill of Rights
You most likely already received the Minnesota Patient Bill of Rights. Please take time to review it. Your well-being is important to us. If you haven’t received this document or would like to have a translated version of it, please ask one of our patient relations representatives.

Financial Arrangements
When you are admitted, we request information about your insurance coverage so that we can bill your insurer. Fairview Health Services will contact your insurance company to verify your policy’s level of benefits if you are admitted as an inpatient to the hospital. This does not guarantee payment by your insurance company. Your insurer may require you to complete additional forms (claimant’s statement, coordination of benefits information, accident reports, etc.) before your claim can be processed. Please respond promptly to any requests from your insurer so payment for your medical charges is not delayed. It is your responsibility to know the appropriate facility and physician or any other requirements for coverage to obtain benefits from your insurance policy. You are also responsible for promptly meeting any financial obligation agreed to with the hospital.

Fairview Health Services has a Community Care program that may provide financial assistance if you are unable to pay the cost of your care and are a United States resident. For more information regarding community care, please call 612-672-5385.

You may receive a separate bill for some professional services. Questions should be directed to the office identified on the actual bill. If you have any other questions or concerns, please contact the Billing department at 612-672-6724.
