Individuals covered by General Assistance Medical Care (GAMC) can enroll in a coordinated care delivery system (CCDS). The University of Minnesota Medical Center, Fairview is one of four CCDS in the state. Along with UMMC, three other hospitals are participating as a CCDS:

-  Regions Hospital in St. Paul 
-  Hennepin County Medical Center in Minneapolis
-  North Memorial Medical Center in Minneapolis

For patients who have chosen the CCDS at University of Minnesota Medical Center, Fairview, care will be coordinated through one primary care site—University of Minnesota Medical Center Riverside Primary Care Clinic. This clinic is room F105 in the West Building on the Riverside campus of University of Minnesota Medical Center, Fairview. The clinic number is
